The Lucas County Family Council with partners like thhe United Way has developed an online data system for 20 agencies in the Toledo area.

Long Term Care Facilities Matrix
Developed in collaboration with the Winter Park Health Foundation, Orlando Florida. The indicators are used to implement and evaluate the Pioneer Network culture change model to improve long term care facilities in central Florida.
