Family Development Matrix, Lucas County, Ohio
The purpose of the Lucas County Service Coordination Plan is to coordinate appropriate, effective and cost-efficient services for children and families; increase family involvement throughout all levels of planning and services; support early intervention with families; and, encourage shared responsibility among systems serving children and families. The Lucas County Family Council with partners like thhe United Way has developed an online data system for 20 agencies in the Toledo area.

First 5 San Bernardino, California
The Children and Families Commission for San Bernardino County (First 5 San Bernardino) was created in December, 1998 in order to realize the benefits of Proposition 10 (California Children and Families Act) for the County's youngest residents and their families. The act created a program for the purpose of promoting, supporting, and improving the early development of children from the prenatal stage to five years of age, under the guidance of the Children and Families Commission for San Bernardino, and in collaboration with the community and agencies providing services to children.

Long Term Care Facilities Matrix
Developed in collaboration with the Winter Park Health Foundation, Orlando Florida. The indicators are used to implement and evaluate the Pioneer Network culture change model to improve long term care facilities in central Florida.
