Collaborative Prevention Plans

The Collaborative Prevention Plan is an organizing instrument to assist design teams to plan and implement the FDM in their communitities. The model is included as well as a step-by-step process. Following are the prevention plans for each FDM collaborative.

Each community network follows a process to integrate the FDM and an implementation plan. The collaborative prevention plan utilizes the resources of the community network to create their vision, values and leadership roles to plan change, overcome obstacles and act as an collaborative to "make things happen" in their community.

Agency Showcases

Each of the FDM agencies has their unique mission, programs, geographical areas, client and cultural populations. We created these showcases with each agency to demonstrate in California the capacity and variety of family resource agencies participating in the FDM. This collection of agency showcases provides a resource for locating innovative programs and a variety of services and their contact information.
